TERMS OF ADOPTION

1.) If the above German Shepherd Dog (GSD) is not already altered, I agree to have the pet altered within 60 (sixty) days of adoption. The Adopter agrees that under no circumstances will the dog be used for breeding, including accidental breeding. If the dog is not neutered/spayed prior to the adoption, the Adopter agrees to have the dog altered. In the meantime; the Adopter agrees to take any and all precautions to keep the dog from having or siring a litter. The Adopter shall within seven (7) days of surgery forward to BrightStar the veterinary certificate of the sterilization.

2.) I agree to keep an identification tag attached to a properly fitted collar which will remain on the adopted animal at all times, whether inside or outside of the house, and to obtain all of the necessary city licenses required by local authorities.

3.) I agree to provide the GSD with all necessary preventative and emergency care including; inoculations, heartworm preventative, dietary prescriptions, wound or bone/muscle/tendon afflictions, etc. at the intervals advised by my veterinarian.

4.) I agree to have the GSD under my control when it is not on the confines of my property. The adopted pet will not be tied or chained. I will keep the GSD as a family member and companion and not a kennel or yard dog. I will never permit the GSD to roam free in an unfenced area without direct adult supervision.

5.) I agree not to abuse or neglect the GSD and I authorize the BrightStar, at their discretion, to determine whether or not the pet has been abused or neglected. I shall under no circumstances physically punish the dog without consulting the BrightStar first as to the appropriate ways of dealing with specific behavioral problems. The Adopter will refrain from physical abuse and any other inhumane treatment and will prevent other adults and children from teasing or physically abusing the GSD.

6.) I understand I will only euthanize the GSD for medical reason only as attested to by a licensed veterinarian, and with the written consent of BrightStar. If the Adopter euthanizes the GSD without the BrightStar consent, "The Adopter" agrees to pay to "BRIGHTSTAR" damages in an equal amount to the value of the GSD as stated in this Agreement, together with any cost and expenses related to the matter reasonably incurred by "BRIGHTSTAR" including attorney's fees. "Euthanizing" the dog, means intentionally putting the animal to death by any method including lethal injection.

7.) I understand that the GSD covered by these adoption papers, is as far as can be determined by the BrightStar in good health and that the BrightStar is not responsible for any medical fees incurred after the adoption date. However, if a health problem develops during the first 10 days, I should notify the Adoptee to discuss the matter. The Adopter assumes all responsibility for treatment of any and all existing conditions and those that may subsequently occur, If the GSD has a known medical problem it is listed as Addendum at the end of this agreement By signing this Agreement The Adopter acknowledges that he or she understands the extend of the medical problem(s) and the recommended treatment specified in the Addendum. While BrightStar makes every effort to provide all medical information no guarantee can be or is made as to the health of any dog offered for adoption and BrightStar shall not be held responsible for any medical expenses that may subsequently be incurred by the Adopter.

8.) I understand BrightStar makes no explicit guarantees regarding the temperament of the GSD. The GSD is adopted "as is" and the Adopter assumes all responsibility for any and all existing conditions and temperament changes that may occur. BrightStar cannot and does not guarantee the temperament of the GSD nor does BrightStar guarantee that the dog is housebroken.

9.) I agree to give written notification to BrightStar within ten (10) days of any change of address, phone or email or any change of circumstances that will affect the GSD, as well as notice of any incident involving police, Animal Control or SPCA.

10.) Regarding transportation of GSD, I agree not to permit the GSD:

a.) to ride in the back of an open pickup truck other than in the secure crate

b.) to remain unattended in any vehicle for any length of time

c.) to be flown in an airplane between 9am and 8pm when the temperature at either departure or arrival airport is expected to be above 22 degree Celsius

11.) I agree to allow BrightStar a reasonable access to the GSD to assure that it has acclimated to it's new environment and it's new family and to ensure the compliance with the terms of this agreement.

12.) I agree The GSD is not to be left unattended with children under the age of fourteen (14) years at ANY time.

13.) I agree that during the lifetime of the GSD, BrightStar retains the right to reclaim the GSD at any time because of neglect, abuse, inadequate care, breach of this agreement, or lack of integration into Adopter's family as determined by the BrightStar. The Adopter agrees to relinquish custody of the GSD immediately upon request. In the event that such action is necessary The Adopter hereby waives any claim for a refund of any kind for any fees and expenses that may have been incurred

14.) I agree that I will not transfer the ownership of the said dog to any other person, firm or corporation without a prior written consent of BrightStar. Furthermore, The Adopter is required to notify the BrightStar Organization within 72 hours in the event of the death of the GSD. In addition The Adopter agrees to notify the BrightStar Organization immediately if the GSD is lost for more than 24 hours.

15.) I understand that BrightStar will provide a lifetime phone and/or e-mail support to help with the integration of the GSD into The Adopter's family and with GSD's well being throughout its entire life.
